| Next Stage Transition Plan |
The LifeGuide Plan ® starts with an in home visit by an experienced care manager (up to 2 hours) followed by research and LifeGuide Plan development (3 ½ hours), with a final ½ hour conference call to answer questions and or clarify plan information. This valuable tool uses a collaborative team approach between clients, families, and health providers. This plan allows you, the health care recipient, to anticipate and meet future care needs in a systematic and organized fashion. After you have had a chance to review the LifeGuide Plan, you will get the opportunity to ask questions and get clarification. Keep the LifeGuide Plan in a safe place as you will refer back to it for months or years to come. |
