Central Intake

Provides streamlined “one stop” shopping for clients and families. Eliminates need for clients and families to keep “repeating” requests to staff.

Intake completed by a Registered Professional Nurse

The Care Manager is able to provide a thorough initial screening in order to identify the client’s needs, make appropriate referrals as needed and provide “courtesy” education to the public regarding home care services.

Quarterly Case Conference

A case conference is an opportunity to evaluate and set goals for ongoing care. The entire care team is invited to participate, including the client and their family. A Proactive approach enables the care team to collaborate with clients and families to ensure care needs are being met and mutually agreed upon expectations are communicated.

Advanced paraprofessional training

Geriatric Care Management services (optional)

Our expertise has prepared us to function as elder care advisors and advocates. Clients and families may take advantage of a holistic approach utilizing quarterly case conferences. Case conferences and care management calls provide care coordination and communication between physicians, family and other members of the health care team.

Next Stage Transition Plan

The LifeGuide Plan ® starts with an in home visit by an experienced care manager (up to 2 hours) followed by research and LifeGuide Plan development (3 ½ hours), with a final ½ hour conference call to answer questions and or clarify plan information. This valuable tool uses a collaborative team approach between clients, families, and health providers. This plan allows you, the health care recipient, to anticipate and meet future care needs in a systematic and organized fashion. After you have had a chance to review the LifeGuide Plan, you will get the opportunity to ask questions and get clarification. Keep the LifeGuide Plan in a safe place as you will refer back to it for months or years to come.
